Reinstall cabinet door hinge - need help please

Does anyone have any ideas on how to rehang my cabinet door. The original hinge has broken through the wood and was about to fall off before I removed it.

And as a second question does anyone know if the original person who installed this should have used an additional screw for extra strength? There's an extra hole on the hinge that I presume is for an additional screw.

Lloyd, In my opinion, it looks to me like this door has suffered some abuse. Perhaps some one leaning on top of the door while it was in the open position. The side wall seems to be a particle board/MDF board. A screw will hold fine in this type of board until presssure is constantly applied and then you have what you ended up with. Is there a hole for a third screw in the back under the adjustment plate?? If there is a third screw hole in the hinge base plate there should have been a screw installed, but I don't think it would have prevented the hinge from pulling away. I really think it was from someone leaning on the door while in the open position.
