Has anyone ever seen maple react like this to staining?

Did you use any sealer under the stain? My first guess is that it is an odd case of blotching. Otherwise, it also looks like something sprayed on the wood prior to staining that affected the stain process.

+1 sprayed on sealer before staining.

Check your door manufacture literature. Some mfg sell unfinished doors with clear grain sealer (for painted doors). If you want stain grade unfinished doors, need to order them without the seal coating.
