Dull tools, missalignment, or taking to deep a cut. Are you cutting with the grain or cross grain?

Changing from going with the grain to across the grain can cause tear out. Also, some oak is more prone to tear out than others. Some dado blades are more prone to tear out than others, and all dado blades are more prone to tear out if dull and/or dirty. Everything mentioned so far is a variable and a potential cause.

You could try using a backer board. I've also used tape on the back of the board with good results. Just don't try using the tape on a board that you will have a cut-off on. Some good tips here. I agree that you should clean the blades first. What brand dado set are you using?
