Someone with more experience may chime in, but one thing might be that you went too deep into the wood portion of the barrel that you contacted the tube.

Time will bring you all your answers. Go slow until you have your skills where you want them. these things happen to those who have been turning pens for many years. Keep going and go slow to learn. Maybe you might benefit from a class from a Woodcraft location and/or a wood group near you. Great job, you finished the project and learn a few things along the way

Kerry, a simple answer to your concerns, its a learning process, put another blank in and go again

Then post the completed work, and very importantly keep your first off, and then as time progresses pull it out, look at it and gauge your progress.
