Maybe all of those factors. Clean the hooks and loops part of the sander with a small wire brush gently. That will help the sandpaper stay in place. Maybe try a different sandpaper, i mean the coarseness of the grit, because it does sound like you are applying too much pressure and creating excess friction/heat that is melting the adhesive. Progress through the different grits, courser to finer, if you are removing a lot of material/wood.

Lastly, if none of that works, try a different brand of sandpaper pads.
