Going by the panel, it looks like glue starvation. Being that the blocks are end grain is another reason why the glue is not holding. Applying glue or liquid nails to the sides of each block would also improve the integrity of the wall.

If I were doing a project like yours, I would glue all the blocks to a sheet of 1/4” plywood and nail into each block with a small brad (nail gun) from the back. The panel could then be glued to the wall and braced until set.
